The mission of Options For Life Foundation is to support eligible applicants within the boundaries of our budget, which is generously sustained by the donations we receive each year. While our financial limitations may restrict us from accommodating every application, we remain committed to fostering hope. Even when our funds are exhausted, or an applicant does not meet our criteria, our dedication to making a positive impact remains unwavering.

After submitting your application, please refrain from contacting OFLF board members for updates. Due to the high volume of applications we receive, our process of assessing eligibility is thorough and requires time. Please allow 2-4 weeks for a response once your application has been submitted.

• Verification of the last two months of rent or mortgage payments

• Documentation of unemployment income, disability benefits, or other income sources

• Pay stubs, IRAs, 401k plans, pension income statements

• Credit Report - Summary page only

• Bank statements for the past two months (checking & savings)

• Tax returns for the previous year

• Recurring disbursements

• Other income sources (spousal support, child support)

• Records of charitable donations in your name (e.g., GoFundMe)

• Digital Assets - End of year statement

• Self-employed Income (including spouse/partner)

• Year-to-date business Profit and Loss statement

• Business bank statements for the last two months

• Business tax return for the prior year
